Le cancer du sein est une maladie fréquente et grave. Ainsi, en dépit d’une prise en charge la plus experte possible dès le début de la maladie, un pourcentage important de patientes va voir apparaître des métastases. Ce constat nous a conduit à réaliser un cours entièrement dédié aux cancers du sein métastatiques, qui s’est déroulé à Nice du 7 au 9 Janvier 2010. Parmi elles, ont été abordés : • L’histoire naturelle et l’hétérogénéité des métastases, les problèmes de l’anatomo-pathologie et des paramètres prédictifs et pronostiques • La radiologie diagnostique et d’évaluation • Les marqueurs sériques • Les cancers métastatiques d’emblée et de la femme âgée • Le problème de la métastase unique • Les traitements loco-régionaux de certaines métastases (chirurgie, radiothérapie) • Les traitements systémiques (hormonothérapie, chimiothérapie, traitements ciblés (anti-HER2, antiangiogéniques, biphosphonates) Sous l’égide de l’AFSOS, les Soins oncologiques de support ont été largement traités. La prise en charge clinique a distingué les diverses identités biologiques et localisations métastatiques. Comme à notre habitude, nous avons consacré la dernière session aux différents référentiels français et étrangers. Nous avons recueilli les meilleures communications des auteurs faisant autorité dans ce domaine, et espérons que cet ouvrage vous sera utile dans votre pratique quotidienne. Highlights: Migraine and epilepsy are distinct disorders with some clinical and pathophysiological overlaps. Rare genetic forms have allowed the identification of molecular/cellular dysfunctions, disclosing pathological mechanisms. Mutations can target the same protein and neuronal networks, but the type of dysfunction can be disease-specific. Common migraine and common epilepsies are polygenic diseases probably caused numerous different mechanisms. Abstract: Migraine and epilepsy are episodic disorders with distinct features, but they have some clinical and pathophysiological overlaps. We review here clinical overlaps between seizures and migraine attacks, activities of neuronal networks observed during seizures and migraine attacks, and molecular and cellular mechanisms of migraine identified in genetic forms, focusing on genetic variants identified in hemiplegic migraine and their functional effects. Epilepsy and migraine can be generated by dysfunctions of the same neuronal networks, but these dysfunctions can be disease-specific, even if pathogenic mutations target the same protein. Studies of rare monogenic forms have allowed the identification of some molecular/cellular dysfunctions that provide a window on pathological mechanisms: we have begun to disclose the tip of the iceberg.

De quelle façon les conceptions et les pratiques occidentales relatives aux maladies transmissibles influencent-elles les comportements d'une autre société ? Dans son étude d'une société africaine, celle des Alladian de la Côte d'Ivoire, A. Caprara met d'abord en évidence les principes qui structurent les représentations de la transmission de la maladie. Il étudie ensuite les transformations entraînées par les contacts avec la biomédecine. Chemin faisant, il souligne les contradictions et les paradoxes qui apparaissent, mais aussi nombre de similitudes conceptuelles. En faisant porter l'enquête sur le rapport clinique entre le malade et le guérisseur, il explore l'espace des faits de transmission, et de contagion, espace qui va bien au-delà des faits de maladie eux-mêmes. Le livre n'en reste cependant pas là et il montre comment, concrètement, la dimension sociale de la maladie et les représentations de sa transmission sont reliées aux connaissances empiriques qui fondent, les conduites destinées à l'éviter. Ethnologie, médecine clinique et santé publique s'entrecroisent dans ce livre sur un thème fondamental pour toutes les sociétés, celui du mal que peut transmettre l'autre et de la façon de l'éviter.
